I give permission for my child(ren) to participate in the JETZ ALL STARS cheerleading and Dance activities. I further agree that I will not hold the JETZ ALL STARS Cheerleading organizers, sponsors, supervisors, volunteer leaders or any participants responsible for injuries or any unforeseen accident while participating in any practice or event, or while traveling to and from or being transported for this activity. I agree to pick up my child (ren) promptly at the time set for the end of the practice/event.
